Elder Care Services

Every family faces unique challenges. With our in-depth knowledge of elder options in southern Maine, we customize our services to fit what you need. We offer a range of options as basic as a simple Advisory assessment, a more inclusive program where we contract services for you, or for more complicated situations, a full Geriatric Care assessment of your loved one’s health, safety and wellbeing. We are not affiliated with any other local agency or organization. Our recommendations are independent and based solely on our clients' needs.

 

Information and Referral Services

When you don't even know the questions to ask...

We know the local services in southern Maine and we know how to assess your situation. We'll help you clarify your needs and goals, discuss options and outline next steps. This session lasts approximately 1-2 hours and includes information on home care, residential facilities and nursing homes.

"Do It For You" Services

When you can't possibly do it all yourself

How will you fit these additional responsibilities into your already busy life? Let us help. We are not affiliated with any local agency or company so we can provide you with an independent recommendation to suit your situation. Each of these options will be customized to fit your need.

  • Home Care
    We can help you understand the differences between agency and independent care. We can make the contacts, set the schedule, hire and oversee the implementation. In other words, we'll make sure it is done to your satisfaction.

  • Residential Facilities
    What are the differences between facilities in southern Maine? Which is the proper choice for your loved one? We will meet with you to determine your needs, then we will identify options and recommend the names of those facilities that we feel best meet you and your loved one's preferences. We will go with you on a tour of facilities if requested.

  • Moving
    Moving from home to a facility doesn't have to be stressful. We can help with the smallest detail and the biggest headache. We'll ensure a successful move and smooth transition.

  • Financial Concerns and Insurance
    Organizing the finances and paperwork is a daunting task for most folks. We can help. Using our Essential Documents Organizer will help you see what vital documents are outdated, missing or needed. We can help you fill out and submit any necessary forms or make calls to secure missing documents. We can work with you and your insurance company to ensure your needs are met as quickly as possible.

Geriatric Care Management

You may not be familiar with the term but a Geriatric Care Manager performs a valuable function for elders and their families. A GCM is an elder care specialist with the experience necessary to provide a comprehensive assessment of your loved one, their health, safety and emotional wellbeing. A GCM often acts as a "professional relative," performing the duties that a son, daughter or other close relative might perform if possible.

There are many reasons to call in a Geriatric Care Manager.

  • If you and your elder are separated by distance or you are a long distance caregiver
  • If someone you know has been diagnosed with Alzheimer's
  • If an elder has no immediate family to make decisions
  • If there is confusion about changes that need to be made
  • If the issues and problems your loved one is facing are becoming too large or complex to manage on your own

Many families and elder individuals can benefit from the dedicated attention of an experienced elder specialist. The Geriatric Care Manager acts as a guide and an advocate. As your GCM, LifeStages will conduct an evaluation of your elder's condition and associated needs. This evaluation will consider the medical, emotional, financial, residential, safety, social and spiritual needs of the key individuals involved. Based on this assessment, a plan of care will be developed to ensure the ongoing highest quality of life possible for your loved one.
